BACKGROUND
The City of Rialto currently utilizes the nonprofit organization Social Work Action Group (SWAG) to provide street outreach services and intensive case management to homeless individuals and families throughout the city.

ANALYSIS/DISCUSSION
On May 20, 2020, City Council approved a professional services agreement with Social Work Action Group, which began on June 1, 2020, and has been approved by the City Council through June 30, 2021 in the amount $294,160. As proposed, services through May 31, 2021 will be funded through a California's Homeless Emergency Aid Program (HEAP) Grant by way of the San Bernardino County Office of Homeless Services (OHS), with the final 7 months of calendar year 2021 being funded by a combination of CARES Act Bill funds through the Community Development Block Grant (CDBG), and budgeted departmental funds.

As of March 31, 2021, four social workers from SWAG have provided services to 194 homeless individuals in the City of Rialto. Those services include but are not limited to: connecting 81 individuals to medical services, 97 mental health services, 35 veterans to veteran assistance and 34 substance abuse treatments. Moreover, 60 homeless individuals received shelter and 28 placed into permanent housing. Overall, SWAG has documented a total of 1,370 interactions with members of the homeless population, wherein varying degrees of service were offered.
